Plan Summary & Benefits

Healthy Any, Any, Any Gold, OH Senior, Medicare Advantage (Part C) Plans Any, Any, Any Gold (H8098-001), Medicare Advantage Private Fee-for-Service Plan, Mahoning County, OH

Any, Any, Any Gold is a Private Fee-for-Service (PFFS) plan for seniors with Medicare benefit living in Mahoning County, OH. With a Medicare Advantage PFFS plan you can see almost any doctor you choose, as long as the provider accepts the plan's Terms and Conditions of Payment. Many Private Fee-for-Service plans combine all the benefits of Original Medicare, prescription coverage and many extras into one simple, easy-to-use plan. If that's what you want, be sure to read all of the details.

The Any, Any, Any Gold Private Fee-for-Service plan works differently than a Medicare Supplement plan. With a Medicare Advantage PFFS plan your doctor or hospital can continue to treat you if it agrees to accept the terms and conditions of payment. For the same reason, they can choose not to treat you (with the exception of emergencies).

The benefit information provided herein is a brief summary, not a comprehensive description of available benefits. Additional information about the plan benefits is available from your agent.
